Thank you for applying to our home repair program! 

In order for us to process your application, you will need to submit the following supporting documentation:

1) Last eight consecutive weeks of pay stubs for each applicant and every family member working in the home.

2) Applicants Bank Statements: past six months of checking statements and past two months of savings statements from applicant only.

3) Documentation of all other household income (e.g. social security, disability, food stamps and child support)

4) Complete Assets Certification Form by clicking HERE

5) Complete Certification of Zero Income Form by clicking HERE

WE CANNOT REVIEW YOUR APPLICATION WITHOUT THESE DOCUMENTS

Following an initial review, we will contact you regarding next steps towards an application for a home repair.
If you do not meet these general guidelines, we will notify you by letter. Please be patient, this review process can take up to 30 days.
